Orientation Location and Time:
Lake Forest-VCA Arroyo Animal Hospital: 6:00 p.m.(the rest of the classes will be at 7pm)
Class Description: Obedience/Problem Solving classes taught one time per week for 7 weeks. Lessons are approximately 1 hour in length (except for the first lesson which is a 2 hour orientation without dogs). MBF uses positive-reinforcement training methods!
I (print name) ________________________________________ wish to enroll my dog in the Basic Obedience Group class training program under the direction of Man's Best Friend, School of Dog Training. I understand the program will cover basic on leash obedience commands: Heel, Auto Sit, Sit/Sit-Stay, Recall (come), Down/Down-Stay, "No", and "OK", as well as help me to solve or curtail my dog's behavior problems. I understand and agree that my success or failure in this program is based primarily on my willingness and ability to work with my dog at least 20-30 minutes every day.
SPECIAL NOTE ABOUT HOUSEBREAKING (POTTY TRAINING):
Most people, by the time they come to class, already have their dogs housetrained and are well on their way to getting their dogs to "go" outside. Because it's something that the majority do not need instruction on, and requires the trainer to stay after class to help the few that still need help in this area, there is an additional $15.00 fee for housebreaking instruction.

Payment is due prior to attending class. Please add $15.00 to your class payment when mailing in your check.

REFUND POLICY, READ CAREFULLY: I understand that all fees are payable in advance to reserve my spot in the class. I understand and agree that there are NO REFUNDS after the first class date.
CANCELLATIONS: I understand that if I cancel my enrollment in the group training program BEFORE the first class date, the class fee will be refunded, BUT will be subject to a processing and accounting fee of twenty dollars ($20.00). It's mandatory to attend the first two lessons (if you miss either of these, you can still be in the class, but will have to do a make-up lesson for $35.00).
